Applications and Approval Forms

This form is for institutions. An institution applying to operate under the State Authorization Reciprocity Agreements (SARA) must submit this form to its Home State's SARA Portal Entity. This same form is used for initial application and renewals. Institutions should indicate on the top right area of the first page if it's an initial application or a renewal application.  

A revised institution application will be posted the week of May 20, 2024, to be used starting July 1, 2024. The revised application will include: the addition of language related to arbitration agreements that was approved during the 2023 Policy Modification Process cycle and has an effective date of July 1, 2024 (PMP23-383); updated language to align with language in the SARA Policy Manual; minor formatting edits; and additional minor edits based on feedback from regional compact SARA staff and State Portal Entities.

This form is for states. A state that would like to apply for or renew membership in the State Authorization Reciprocity Agreements (SARA) must submit this form and required documentation to its regional education compact’s SARA office.
